Dr. Matthew Abinante, DO, MPH

Board-Certified Family Physician (not accepting new patients, waitlist available)

Dr. Matt had a few health issues when he was younger and got to work with several doctors that became role models to him. He loves working with people and is fascinated by the human body. Family medicine allows him to follow in the footsteps of his role models in providing excellent care while working with people of all ages in their quest for good health.

Dr. Matt received his DO and MPH from Touro University in Vallejo, CA and completed his residency training at PIH Health Hospital in Downey, CA. He is a board-certified Family Physician.

Dr. Matt lives in Huntington Beach with his wife, Chelsea, and their four daughters. In his spare time, he enjoys spending time with his family whether it's at the beach, his parents’ house in Sacramento or with his wife’s family in San Diego. He also loves spending time on his grandparent’s ranch in Idaho.

He speaks fluent English and Tagalog.


Dr. Paul McKinley, DO

Board-Certified Family Physician

Dr. Paul, a native of Southern California, has been in practice since 2002 and joined Elevated Health after serving as a medical director in Chicago for Iora Health, a nationwide, innovative, direct primary care organization. Prior to his time in Chicago, he spent 13 years in private practice in Orange County taking care of patients in both the office and hospital settings.

Dr. Paul practices the full spectrum of primary care medicine managing all health conditions and is skilled in outpatient medical procedures. He emphasizes a preventive approach to health with all of his patients. As a former college basketball player and athlete he enjoys the field of sports medicine. He has also completed a training program in the science of longevity medicine which aims to prevent the chronic diseases of aging and increase healthspan (the period of life in which you are healthy). This is achieved using a combination of nutrition, exercise, medications, nutritional supplements, and hormone replacement, if necessary.

Dr. Paul loves practicing in the direct primary care model where he can spend time with patients and develop relationships with them. This allows him to deliver a personalized approach to each individual patient and have the opportunity to help them live healthier and more vibrant lives.

In his spare time Dr. Paul loves being in the outdoors with his wife Kate, and son.  He especially enjoys the mountains, where he has climbed Mt. Rainier twice, and can be found snowboarding every winter.


Dr. Lucy Pun, DO

Board-Certified Family Physician

Dr. Lucy Pun has always loved being the medical caretaker of her family.
Eventually, that passion motivated her to pursue a career in medicine and
take care of many other families. She enjoys taking care of people of all ages and considers her patients as part of her family. Dr. Pun chose Direct Primary Care because it allows her to spend quality time with her patients, provide education, and empower her patients to be in charge of their

Dr. Pun received her Bachelor of science at UC Irvine. Prior to starting medical school, she trained in child development and was a Montessori preschool teacher. She then received her medical degree from Western University of Health Sciences, and completed residency at PIH Health Hospital in Downey. She is board certified in Family Medicine.

In her spare time, Dr. Pun enjoys traveling, watching movies, and spending
time with her husband and her son.

She speaks fluent English and Armenian.


Ivette Adame, RDMS

ultrasound techniciAN

Ivette has been in the medical field since 2012. She has worked everything from urgent care, family practice, hospitals, to mobile Ultrasound companies. Her work has allowed her to travel all of California for work. She is now overjoyed to join our team here at Elevated Health.

Ivette knew she wanted to be an ultrasound technologist the moment she saw her younger sister up on the ultrasound monitor when she was 15 years old. In pursuit of her dream, she graduated from a Diagnostic Medical Sonography program. She completed her training at Bellflower medical center in the city of Bellflower.

Ivette lives in Garden Grove with her Husband, Mark. In her spare time, she loves spending time in Laguna Hills visiting her parents and younger siblings. She also has a passion for music (mostly reggae) and loves to see live bands with her husband. 

She speaks fluent English and Spanish.

Danny Gutierrez, RN

Registered Nurse

Danny received two Associate Degrees from Cerritos College, CA, in the fields of Nursing and Communication Studies. Education is a lifelong journey for Danny. He is currently working on his Bachelor’s Degree of Nursing (BSN) at The University of Texas at Arlington, TX.

Healthcare has always been his calling. He started 7 years ago as a Medical Assistant and now has entered this new chapter of his life as a Registered Nurse. He takes this proverb with him, “Do what you can see yourself doing in a world where money didn’t exist.” That world is healthcare, Nursing.

Danny currently resides in Norwalk with his wife. In his spare time, Danny is dedicated to his church, taking on the leadership roles of minister, youth leader and lead musician. On any given day he can get a smile from you with his impressive Stitch character voice.

He speaks fluent English and Spanish.


Daniel Zike, BS

Office Manager

Daniel has worked in healthcare for nearly 10 years. He graduated from Cal State University, Long Beach with his Bachelor degree in Healthcare Administration. He has worked in a lot of diverse healthcare settings including a rural health center, psychiatry, urgent care, and family practice. He has spent the last three years at Elevated Health.

Daniel currently lives in Long Beach with his wife Shannon and their two fur babies Haydes and Detective Mittens. They are expecting their first child in February 2020. When he isn't working, he enjoys a nice IPA (or three), traveling, and following sports.


Fun Fact: Daniel once raised a bull-frog from a pollywog he found in a lake for 6 months. His name was Homer. Be sure to ask him about it!


Dr. Stephanie Jacobs, Ph.D.


Dr. Steph has a doctorate in Counseling Psychology from Purdue University. She has worked in university counseling, primary medical care, cancer treatment, and in private practice. Her areas of expertise are anxiety, depression, PTSD, death/dying/bereavement, chronic pain, and chronic health issues. She will help you develop effective coping strategies, manage chronic health conditions, and improve your quality of life. Dr. Steph is passionate about her work and looks forward to meeting you!

Dr. Ben Pun, D.C.


Dr. Benjamin Pun completed his Bachelors of Science degree at University of California, Irvine and his Doctor of Chiropractic degree at the Southern California University of Health Sciences where he graduated with Magna Cum Laude honors.

Dr. Ben uses a variety of gentle and effective hands-on chiropractic techniques including Diversified, Activator, Webster, and Cox-Flexion Distraction Technique. In addition to treating acute and chronic conditions, he also has experience treating an abundance of sports related injuries. He was a member of the Sports Injury Council and has volunteered his chiropractic services in various sporting events such as the Wildflower Triathlon and Surf City Marathon.

Dr. Ben is so passionate about improving the health of entire families that he extended his education to include pregnancy and pediatric chiropractic treatment which has become his specialty.


Heather Baltazar, LVN


Heather has been an LVN since 2005. She has worked in hospice and home health, registry positions, flu clinics and wellness centers. Prior to joining Elevated Health, Heather was a Nurse Manager at an outpatient clinic in Torrance, CA. It was in this position that she found where she wanted to be in nursing - in a healthcare model where the patient is the emphasis and the cause. She brings those years of diverse experience and passion to the Elevated Health team. 

Heather lives in Long Beach, CA with her husband and their dog, Mister Toast. When she’s not working, Heather likes to read, travel, catch up on TV and spend time with her family and friends. You can usually catch her on her phone or computer looking up travel destinations. Her goal is to keep adding stamps to her passport!


Noelle Capulong, PA, Addiction Medicine

Physician assistant, addiction medicine

Noelle is a Huntington Beach native who became interested in medicine after volunteering at the Huntington Beach Community Clinic as a high school student.  After college, she attended Western University of Health Sciences and obtained her Physician Assistant license.  She has worked in family medicine, women’s health, pain management, and substance abuse research but has special interest and training in addiction medicine.  She is very happy to have joined the Elevated Recovery team where she hopes to make a difference in patients' lives.  Noelle lives in Huntington Beach with her husband, Leo, twin daughters Chloe and Michaela and her beloved lab-mix rescue, Ruby.  In Noelle’s spare time, she can be found at dog beach, at the gym or at the movies.


Jennifer Speer

Marketing Manager & Patient Advocate

Jennifer recently moved here from Denver, Colorado to join the Elevated Health team. She brings a wealth of marketing and social media skills with a passion for company brand image and growth. As Marketing Manager and Patient Advocate, she runs various operations including client outreach, website management, DPC networking and advertising/marketing efforts. In her free time, Jennifer enjoys bicycle riding, surfing and staying active in beautiful Southern California.


Jessica Mason

Front desk administration

Jessica was born and raised in San Diego, CA., and recently moved to Huntington Beach to join our Elevated Health family. Jessica attended Brigham Young University in Utah, and graduated with an emphasis in Family and Consumer Sciences. Prior to her running our front desk administrations, Jessica worked at a wellness center. She has always loved the daily interactions with patients and enjoys working in the medial field. In her spare time, she can be found with her husband Chase, either baking cakes, playing pickleball or eating her favorite food, Tacos.
